Privacy Policy: May 2018

Data Controller: Charlotte Lawrence-Smith

As an independent SLT I retain a certain amount of information about clients for the purpose of Speech and Language Therapy.

The information held about clients will include:

  • Name and date of birth
  • Name of parents/carers
  • Address and e mail address
  • Telephone numbers
  • School/educational address and contact details
  • Referral source
  • Initial case history
  • Professional reports/EHCPs
  • Assessment reports
  • Notes
  • Client targets and termly reviews

I record details of medical and developmental history as appropriate and necessary for assessment and therapy. The information gathered is for this specific and legitimate purpose only. It is limited and relevant to therapy.

The information is confidential. It will not be shared with a third party without explicit consent unless needed for child protection purposes.

Information will be kept for as long as is needed to fulfil our professional obligations through the Health Care Professional Council (HCPC).

Your details will not be sold to a third party.

Information is securely held on devices including laptops, IPADs and phones which are both encrypted and password protected.

Hard copies of information are kept securely in locked cabinets within a locked building. As part of my work involves school and home visits, some information is transported for these visits. Only data that is needed for that visit is carried and care is taken to ensure its security.

Videos may be taken of clients with parental consent. These are temporarily stored on an encrypted and password protected tablet. These are viewed for the purposes of planning therapy and writing notes. The video is then deleted.

Emails are sent via Outlook, which may not be 100% secure. If you would prefer that information is not sent via this platform, please do let me know. Sensitive information sent via e mail is always password protected.

I am registered with the Information Commissioner’s Office (ICO) as a Data Controller. In the event of a breach of data security I will notify clients and the ICO immediately as I become aware of this.

You have a right to withdraw your consent to the retention of your data. You must give this in writing. You also have the right to request a hard copy of information we hold about your child as well as the right to request your record to be amended if you believe this is wrong. I will provide access to your child’s records within 30 days of receipt of all necessary information.
