Enquiry and Booking

  • Initial enquiries (telephone, email and face to face) of up to 20 minutes will be free of charge.
  • Booking for assessment or intervention must be made directly with Charlotte Lawrence-Smith.
  • 7 days prior to assessment taking place, parents will need to provide all relevant information and available documentation regarding the child’s circumstances and condition (information questionnaire, other professional’s reports).

Assessment Process

  • If the client has not been assessed by Charlotte Lawrence-Smith before, a screen assessment is always recommended so the client will be provided with a clear baseline. This will provide up to date assessment of the client’s needs and is in the client’s best interests.
  • A screen assessment includes an initial assessment lasting one hour including some standardised and non-standardised assessment, a full Case History and structured observations.
  • A full SLT assessment includes an initial session lasting approximately two hours, (depending on complexity) including standardised and non-standardised assessment tools, questionnaires, discussion and structured observations.
  • A summary/detailed report of assessment findings will be sent to the client within 2/3 weeks of the assessment. Following this, a 20-minute phone call is offered to discuss questions and recommendations.
  • In some circumstances it may be necessary to assess the client over several sessions or in different environments to gather sufficient information.

Treatment Policies

  • Please support Charlotte to ensure chargeable cancellations are kept to a minimum by ensuring all family and staff involved in the client’s care are aware of their responsibilities to communicate cancellations as soon as practically possible. Should the client be unable to attend a prearranged appointment, at least 24 hours’ notice is required other than in exceptional circumstances. If the client or instructing party is unable to give 24 hours’ notice, you will be charged the full fee for the missed session.
  • Charlotte Lawrence-Smith reserves the right to cancel assessment or treatment sessions if she is ill.
  • Any consultation/ administration outside of the allocated sessions will be charged in line with the pro-rata hourly rate for professional time (e.g. inter-professional liaison, telephone consultations, etc.).

Documentation / Liaison / Privacy

  • It is in the best interests of the client that other professionals involved in the care of the client are kept up to date with progress and intervention given by Charlotte Lawrence-Smith. It is the client’s responsibility to make Charlotte aware of any other professionals involved with the care of the client. It is important that all professionals working with your child can work together, communicate and share reports and progress documents. Your permission for information to be shared with professionals or other relevant parties is required prior to this being shared.
  • Any information shared by the client with Charlotte is treated as confidential unless a safeguarding concern is raised.
  • Please read Charlotte Lawrence-Smith’s Privacy Policy in conjunction with this document before signing the Terms and Conditions.

Payment

  • All fees due for therapy services provided by Charlotte Lawrence-Smith are the client’s responsibility and need to be paid either before/or on the day of the session.
  • Where payment is via an identified third party, payment is expected within 5 days of date of invoice. It is the responsibility of the client / parent to ensure this agreement is in place prior to the commencement of therapy.
  • If payment is not received prior to, or on the day of assessment, or if an agreed date of payment be exceeded, reports will be withheld until outstanding payment is received.
